AMWA Housing Request
Dear interviewee,
In order to plan your stay, we need to have your request and payment at least 10 days before your scheduled interview. UTMB will provide an area for you to leave your belongings on the morning of the interview. Please note that limited housing will be available for those students interviewing on September 11th due to a scheduled vacation for the 2nd year medical students.
The fee for housing is $20 if paying online. Add $4 processing fee if sending payment via US Postal Service. Any requests for refunds will not be granted unless the request is made at least 24 hours prior to your expected arrival date.
Immediately after submitting your form, you will be redirected to a page that explains your payment options.
You will not be matched for housing until your payment has been received.

**Remember, this form is a request for staying with a medical student. If you wish to stay in a hotel or fraternity house, you must make arrangements on your own.**
